2022-2023 ONLINE ATHLETIC PACKET

An Athletic Packet must be submitted online to participate in a sport during the school year.

NEW ATHLETES: Create an ATHLETIC CLEARANCE account to serve as an online Athletic Packet.
TRANSPORTATION WAIVER: REQUIRED! Once completed you will receive an email confirmation which will serve as your proof that you will need to upload into the ATHLETIC CLEARANCE.

RETURNING ATHLETES: Log into your existing ATHLETIC CLEARANCE account using the same username/password credentials you had previously. Use the “Start Clearance Here” button for 2021-2022. Please check if your physical has expired. Physicals are good one year to date.

PHYSICALS: To submit proof of a physical exam, upload the CHS/LVUSD ATHLETIC PARTICIPATION HEALTH FORM to your Athletic Clearance account. Please make sure the physical is signed and stamped by a doctor’s office. Athletic Physicals can be completed by any doctor or urgent care facility

CLEARED/UNCLEARED: After you submit the Athletic Packet, our Athletic Department reviews the contents before "Cleared" status can be granted. Once your packet has been reviewed and "cleared" the athlete will get an email saying that this step has been completed and the athlete is all set and "cleared" for the season.
